Ventricular Assist Device (VAD): Supply Checklist and Home Setup

Home Supply Checklist:

  • Running System Controller

  • Backup System Controller

  • Batteries (8)

  • Battery Clips (4)

  • Battery Charger (1)

  • Mobile power unit (MPU) (1)

  • Patient pack (1)

  • Shower bag (Will be provide in clinic once showering is allowed)

  • Back-up bag

  • Dressing supplies

Instruction for setting up at home:

  • Set up a VAD station (you can use a desk, bureau, end table, etc.) where you keep the following:

    • Battery charger

    • VAD binder and HM3 patient manual

    • Dressing supplies

    • Any extra VAD supplies, bags, etc.

    • Flashlight in case of power outage

  • Set up a medicine station where you keep the following:

    • Medicine list

    • Medicines 

    • Pill box

  • Place refrigerated medicines in refrigerator

  • Always keep medicines and syringes out of reach of children

  • Save important phones numbers to your phone:

    • VAD Program: 267-426-2882, for appointments, refills, non-urgent questions

    • VAD Phone: 267-844-3653, for immediate or emergency concerns, VAD alarms, and any issues on nights and weekends

    • 911: for any life-threatening emergencies
