Ventricular Assist Device (VAD): Clinic and Hospital Checklist

What to bring to Ventricular Assist Device (VAD) Clinic:

  • Home VAD flowsheet: you may bring the actual copy or a picture of the flowsheet.

  • Current medicine list:

    • Bring all medicines (except Lovenox® and syringes) to your first appointment.

    • If you would like help filling your child’s pill box, bring medicines to all appointments.

  • Equipment: 

    • Running system controller with 2 connected batteries secured safely in your VAD accessory bag.

    • Emergency VAD bag with back-up system controller, back-up battery clips, and 2 extra batteries.

What to bring if your child is being admitted to the hospital:

The hospital will provide a battery charger and mobile power unit (MPU) to be used while in the hospital. You will need to bring:

  • Running system controller with 2 connected batteries secured safely in your VAD accessory bag.

  • Emergency VAD bag with back-up system controller, back-up battery clips, and 2 extra batteries.
