Transfers: Two-Person Dependent Floor Transfer
These instructions describe how two caregivers of a Children's Hospital of Philadelphia (CHOP) patient can safely lift their child to or from the floor.
Instructions: wheelchair to floor
Preparation

Place the wheelchair next to the transfer surface.

Lock the wheelchair brakes.

Remove the footrests and the armrest on the side of the transfer.

Unfasten the seat belt and any other supports.

One caregiver stands behind the child and supports underneath the arms. The second person supports under the child's thighs.

Another option is for both caregivers to lift from the side. One caregiver can support the trunk and buttocks, while the second supports under the child's thighs.
Lifting

Both caregivers lift the child upward as they stand fully upright.

Both caregivers step to clear the wheelchair, then lower slowly until the child is safely resting on the floor.

Preparation

Place the wheelchair next to the child.

Lock the wheelchair brakes.

Remove the footrests and armrest closest to the child.

Unfasten the seatbelt and hang it from the sides of the chair. Do not place it on the seat.

Instruct your child to cross their arms on their chest. One caregiver squats behind the child and supports underneath the child's arms. The second person squats in front of the child and supports the child's thighs.

Another option is for both caregivers to lift from the side. One caregiver can support the trunk and buttocks, while the second supports under the child's thighs.
Lifting

Both caregivers lift the child slowly and stand fully upright.

Continue to lift the child into the wheelchair, lowering slowly until the child is resting on the seat.

Position the child properly in the chair, buckle the seat belt, and secure any additional supports.

Clear any obstacles from your path before performing this lift.

Do not perform the lift if you are not sure you can complete it safely.
